Second-Generation Anti-H1 Antihistamines
Single dose per day (conventional dosage) for adults or the pediatric population.
The indication for long-term treatment should be considered based on the severity of symptoms, frequency of episodes, and impact on quality of life (evaluated by scores such as UCT or DLQI).
In some cases, exposure is rare and predictable, and a temporary preventive treatment can be implemented.

In a preventive setting or as a first-line long-term treatment, the working group recommends second-generation anti-H1 antihistamines at conventional dosage (single dose, according to marketing authorization) as the first-line treatment for cold urticaria. They are effective and well-tolerated. There is no argument to favor one molecule over another.
Only the single dose of anti-H1 has marketing authorization in France.
Rupatadine, bilastine, desloratadine, cetirizine, mizolastine have been studied at single dose for cold urticaria.
